If you are currently awaiting a decision for an Education Health and Care plan, or maybe you are awaiting your draft for a transfer review. You need to be fully aware of what evidence is being gathered.

Health evidence is currently your last clinic letter, yes you heard right! your last clinic letter of your short update appointment is being used as health input in to your EHCP so you must be aware that this is happening so you can ensure during clinic appointments your child's consultant knows everything and writes it in that update letter.

Please remember your transfer review is a FULL reassessment unless you agree its not needed so beware during annual reviews in school because you may agree without realising.
